This app is meant for United States licensed clinicians treating patients for conditions including but not limited to:

Sprains and Strains

Nosebleed

Sinus Problems

Urinary Problems

Clinicians may have a video or phone visit with a patient and chart medical history, diagnosis, medical decision making, provide excuse from work/school, etc. If medically necessary, the provider may prescribe the patient medication(s).Patients may review their chart in the patient app, please search for CH Care OnDemand.

The purpose of this app being distributed through the store is to allow CHS clinicians and non-employee or contracted clinicians to have accessibility to connect with patients throughout the United States. Non-employee clinicians are not part of the CHS Buffalo Health System and therefore, do not have access to internally distributed systems and may not be authorized users on the CHS secure network. Clinicians - If access has not been set up yet, please contact the help email in the app to obtain secure credentials.
